What problem it solves
Providing equipment and being able to show you provided it are separate things. The register is what closes the gap when someone asks for evidence.
Who it is for
Anyone responsible for issuing protective equipment on site.
What the document covers
- Employee name and role
- Equipment type, size and quantity issued
- Issue date and replacement due
- Employee acknowledgement signature
- Condition and replacement record
How to use it
Sign at the point of issue. Retrospective completion produces a register that is technically complete and evidentially worthless.
Limitations to be aware of
Issue is not use. The register shows equipment was provided, not that it was worn or that it was the right selection for the hazard.
This is general information. A template provides a structured starting point and does not by itself create legal compliance. What your organisation and site require comes from the applicable legislation and a competent practitioner.
