What problem it solves
A file with the right documents in no discernible order still fails review, because the reviewer cannot find anything. An index is what turns a folder into a file.

How to use it
Fill it in as the file is assembled rather than at the end. Used properly it also functions as a gap check: an empty row is a missing document.

The index records what is present. It does not verify that the contents are correct, current or adequate for the project.
