Permit systems fail at the two ends: issued without the precautions actually verified, or never closed so the permit stays live after the work is done. The document structures both.

Limitations to be aware of
A permit authorises work under stated conditions. If conditions change the permit is void and needs reissue, not amendment in the margin.
