Respond safely first
Arrange emergency assistance, prevent further harm and preserve the scene where safe and lawful. Complete required notifications without delaying urgent care.
Gather time-sensitive evidence
Record conditions, photographs, measurements, equipment state, documents and witness accounts promptly. Separate observation from assumption.
Look beyond the final unsafe act
Examine planning, design, supervision, competence, workload, maintenance, procurement, communication and change. The last action in the sequence is rarely the only cause.
Choose corrective actions that control causes
Prefer engineering, design and system improvements over reminders alone. Assign owners, resources and realistic completion dates.
