Record construction defects and nonconformities with requirements, evidence, containment, cause, authorised correction and verification.
State the requirement
Reference the drawing, specification, procedure or acceptance criterion and describe the actual condition and location.
Contain the problem
Prevent covering, further use or repetition while significance and disposition are assessed.
Authorise the response
Repair, rework, accept under concession or reject only through the defined authority and competent technical review.
Verify before closure
Inspect the correction, attach evidence and consider cause and recurrence. A signature without verification is not effective close-out.
