Use occurrence books as controlled chronological evidence with accurate entries, clear corrections, escalation references and effective shift handover.
Record facts in sequence
Enter date, time, location, people involved, observation, action and notification. Distinguish what was seen from what another person reported.
Preserve integrity
Do not erase or remove entries. Correct errors transparently according to the organisation’s procedure and protect completed books from unauthorised access.
Reference related evidence
Link incident reports, access records, photographs, CCTV references, keys, permits and supervisor instructions using traceable numbers.
Use the book during handover
Review open issues, unusual events, equipment status and pending actions with the incoming shift. Supervisors should sample entries for quality and follow-up.
